FQHC look-a-like sliding fee

ValleyWise Health, formerly known as Maricopa Integrated Health System is a Federally Qualified Health Center look-a-like that provides primary care, OB/GYN, pediatrics, dental, behavioral health, pharmacy, WIC and wellness programs. They accept individuals on AHCCCS, have a sliding fee scale for uninsured patients and accept private insurance.  They are a Well Woman HealthCheck provider. They receive county funds to provide care which by default makes them the county system and operate a hospital along with outpatient specialty services.

Eligibility

Each FQHC has their own eligibility criteria. Most dental clinics prefer you be a primary care patient to be seen for services.
