Most, if not all of the FQHC and FQHC look-a-like clinics have eligibility specialists that help people apply for insurance.  This can also be done at any DES office and there are a number of other independent agencies like Keogh that also perform this function.

If the process is completed at an FQHC and the person does not qualify for insurance or are only approved for Emergency aid then they will be able to take advantage of the clinic’s sliding scale depending on their income level and will be eligible for discounted services like office visits, labs, medications, vaccines, etc. for the next year.

In order to submit a application the person will need to bring:

Proof of identity

Income for the last 30 days

Address

Proof of dependents
