Regardless of whether you practice by the JNC8, USPSTF, Sprint trial or AHA/ACC guidelines there are a number of resources available for treatment and monitoring of blood pressure. When working with the underserved remember not to hold their medications hostage based on unnecessary visits. If their blood pressure is controlled give 1 years worth of refills to help decrease unnecessary emergency room visits for medication refills. Like any chronic disease there are a number of facts that may be missed by the patient which is why a patient plan of care has been provided to help your patients understand and take control of their hypertension. Also available are educational posters, and videos for providers and patients.

Medications

The $4 list is a great resource for first line hypertension medications. It also includes non-first line medications that would be reserved for patients with active coronary artery disease, congestive heart failure, resistant hypertension, etc.

Point of care labs

The National Association of Free Clinics has multiple partners each year that sponsor establishing point of care labs in free and charitable clinics that include but are not limited to A1c, istat, lipids and microalbumin which would cover all of the laboratory based quality metrics for patients with hypertension.
